WHAT YOU WILL WORK THROUGH

  • Create a telehealth contingency plan tied to the client’s current location.
  • Write a concise note separating report, observation, formulation, and plan.

The essential idea

Technology changes access and introduces new questions about privacy, accessibility, identity, location, connection failure, and emergency response. Choosing a convenient platform alone does not establish that a service is appropriate. Confirm scope, jurisdiction, organizational requirements, informed consent, and a workable contingency process through qualified supervision.

Documentation should support continuity and explain the work accurately. Separate the person's report, observable behavior, your assessment, and the agreed plan. Common note structures organize these elements differently; no acronym automatically makes a note complete or legally sufficient. Include relevant clinical reasoning without unnecessary personal detail.

A fictional note stating “client was manipulative” offers a judgment without showing the observation. A more useful learning exercise identifies the reported or observed event, the context, and the reasoning it informs. Do not copy fictional examples into real records or use a study tool to store client information.

Caseload management includes timely follow-up, risk-sensitive prioritization, appropriate consultation, and planned transitions. A referral should account for availability and accessibility, consent for information sharing, and continuity. Storage, retention, disclosure, and correction rules require verification in the actual setting.

The clinical frame crosses the screen

A telehealth visit includes a person, location, communication channel, privacy conditions, and an emergency response pathway. Verify identity and current physical location, suitability, consent, applicable jurisdiction and organizational requirements, and who else may be present. Being licensed where the clinician sits does not by itself establish authority where the client is located; requirements must be checked for the actual case.

Agree what happens after disconnection, including callback, local resources, and an authorized support contact where appropriate. A list of national numbers is not a location-specific emergency plan. Accessibility includes hearing, vision, language, devices, data, and a place to speak; video is not inherently more accessible than in-person care.

Write the reasoning needed for continuity

SOAP separates subjective report, objective observation, assessment, and plan; DAP groups data, assessment, and plan. The format matters less than traceability. Record what the client reported, what you actually observed, a supported formulation with uncertainty, the intervention and response, and the next agreed action. Avoid describing unobserved posture or behavior outside the camera frame.

Original example: “Client reports two nights of poor sleep after a schedule change. Speech audible and coherent during the connection; lower body not visible. Explored sleep routine and agreed monitoring; cause remains uncertain. Client identified a feasible bedtime cue. Review next visit.” This is more useful than “anxious, processed issues, continue.” Document access failures and limitations when they affect the encounter.

Minimum necessary, accurate, and reviewable

Clinical records should serve care, accountability, and lawful requirements without becoming an unnecessary transcript of intimate details. Record relevant risk reasoning and actions accurately; do not insert a stock denial or examination finding you did not assess. Handoffs require an appropriate authorization or other valid basis and a clear receiving process.

The HHS sources are implementation guidance, not a complete statement of every jurisdiction’s law or an endorsement of a platform. Check current professional, legal, privacy, and organizational requirements in practice. Classroom exercises use invented data and should never invite real client records.

FROM CONCEPT TO DECISION

A visit from an unexpected location

FICTIONAL CASE

Dev, 40, joins from a parked car while visiting another state. The clinician’s record lists Dev’s home address. Audio intermittently drops. Dev wants to discuss work stress and denies any immediate emergency when asked.

  1. Re-establish the frame

The saved address does not describe today’s location; authority and privacy need checking before proceeding.

“Before we continue, where are you physically today, are you safely parked, and can anyone hear us?”
  2. Resolve suitability

Follow jurisdiction and organizational guidance; agree whether to continue within authority or arrange an alternative. Confirm callback and local contingency information.

“Let’s agree what happens if the connection drops and make sure this visit is appropriate from your location.”
  3. Document limits

Record current location, connection limitations, consent/suitability decision, actual content assessed, and follow-up.

“We covered the immediate plan; the interrupted assessment will be completed at the next appropriate visit.”
What the case shows

Continuity requires an accurate record of what happened and what remains. A familiar account and green connection icon are insufficient verification.
